Must haves to be considered: 1.) Must be a board certified or board eligible medical doctor 2.) MUST have a fellowship in pain medicine 3.) Must have DEA at time of start 4.) Must have a minimum CPR or BLC certification *Will license the provider with a Florida medical license. Base Salary: $270k-$325k Incentive/productivity program Some MD's make over $800k per year. Location: Lake City, FL 2 clinics within 10 miles apart Schedule: Monday - Friday 8am - 5pm NO CALL Cases/procedures: Epidural steroid injections Spinal cord stimulators Sympathetic blocks Trigger point injections Pain pump Support staff: 1-4 mid-levels 1-2 medical assistants 1 Market liaison Full Staff Credentialing: 3-6 months Depends on State license and enrollments Interview process: 1.) Phone Screen 2.) Phone interview with Medical Director 3.) Need document of past 6 months case log 4.) Onsite visit
