Nurse Practitioner - Monroe County

Overview

This position is in Rochester, NY Required skills & experience (the “must haves” to be considered) 1. Active, unencumbered RN and NP license in the state of practice 2. National board certification in adult, family or geriatric specialties 3. Has an active DEA license 4. Current BLS for medical provider 5. Must have valid state driver license and automobile insurance 6. Strong computer skills with knowledge working in an Electronic Medical Records system (will be trained on their EMR system) 7. 2+ years' experience in care of adult, chronically ill patients preferred 8. 6+ months of Nurse Practitioner experience 9. Must be willing to travel in 50-mile radius from primary residence 10. Experience in Primary care, urgent care, home health, ER, critical care (**Do not send Psych, Peds, or Ortho candidates. They won't look at them**) What you need to know 1. Nurse Practitioners are responsible for providing health care to patients in their home. 2. CareSight line of business. 3. These patients have multiple visits to the ER, and the goal is to decrease the re-admittance rate 4. Completes proactive, in-home comprehensive visits 5.
