Overview

Required skills & experience (the “must haves” to be considered) 1. Holds active, unencumbered license in state of practice 2. Credentialed in adult, family, or geriatric care 3. Has active DEA license 4. Experience in care of adult, chronically ill patients 5. Two or more years of practice experience, nursing home or assisted living experience (LTC, STC, ALF, etc.) 6. Driver’s license without restrictions and good driving record 7. Working knowledge of computers and ability to document effectively and efficiently in an electronic system What you need to know • Delivers cost-effective, high-quality care to assigned patients • Completes comprehensive history and physical examination on all enrolled patients in an in-facility visit • Completes HRA (Health Risk Assessment) for all enrolled patients annually • Collaborates with RNs, NF care team, family and patient to determine patient’s risk of adverse health outcomes • Adjusts the patient’s risk level as appropriate based on assessments and performs patient visits **Strong benefits** 3 weeks PTO 401k match Life insurance The NP will be provided with the following: Cart completed with meds, equipment, etc Laptop with built in wifi EMR system - will be trained on EMR
