Nurse Practitioner

Overview

Required skills & experience (the “must haves” to be considered) *Active, unencumbered RN and NP license in the state of practice *National board certification in adult, family or geriatric specialties *Active DEA license *Current BLS for medical provider *2+ years practice experience, nursing home or assisted living experience preferred *Strong computer skills with knowledge working in an Electronic Medical Records system *Experience in care of adult, chronically ill patients What you need to know • Delivers cost-effective, high-quality care to assigned patients • Completes comprehensive history and physical examination on all enrolled patients in an in-facility visit • Completes HRA (Health Risk Assessment) for all enrolled patients annually • Collaborates with RNs, NF care team, family and patient to determine patient’s risk of adverse health outcomes • Adjusts the patient’s risk level as appropriate based on assessments and performs patient visits **Strong benefits** 3 weeks PTO 401k match Life insurance The NP will be provided with the following: Cart completed with meds, equipment, etc Laptop with built in wifi EMR system - will be trained on EMR
