This will be a remote/ work from home position but will shadow and go on home visits with the Outreach Specialist 3. Help manage members to provide effective and efficient service coordination 4. Oversee on-site/in-home member assessments for safety risk, health needs, and barriers to care 5. All mileage is expensed to company
