Overview
Required skills & experience (the 6 “must haves” to be considered): 1. Board certification as an MD or DO with a current unrestricted MI license to practice medicine is required 2. Active DEA 3. No required experience. There will be a lot of room for training and development. 4. Hands-on skills needed but not a deal breaker - training will be provided. 5. Board Certified in Family practice, primary care, specific training in gerontology or internal medicine. 6. Must have reliable transportation with the ability to travel locally up to 30 miles/day What you need to know: • Value-based model – no fee-for-service visits; this program is free for the patients • The team will provide direct primary and urgent care services in the patients’ homes to include: Acute care, chronic disease management, trouble-shooting and maintenance of ventilator, trach, pic line, peg tube, etc, procedures such as wound care, laceration repair, simple suturing, mobile EKG, mobile imaging, medication management, etc, referrals and other duties as needed. • Patient demographic: Medicare Advantage patients within the top 30 percentile for risk of readmission to hospital • Hub located in Detroit metro area - house calls up to 30 min drive radius • 100% clinical role with admin time for documentation • Looking for someone to start mid-late march to begin training/onboarding needs • Formulate comprehensive care plans taking into account patients' complex chronic conditions plus social and environmental factors
