RN Case Manager - Home Health/Hospice - PRN

Overview

MUST HAVES/JOB DETAILS: • Valid, Registered Nurse license for the state of MI • Minimum of one year nursing practice or equivalent experience • Valid Driver’s License and auto liability insurance • Flexible to work some nights and weekends • Traveling around the local area to care for patients, no overnight travel though PREFERRED/NICE-TO-HAVES: • Hospice • Home Health • Oncology • Long-Term Care • Post Acute, ICU, Emergency WHAT YOU NEED TO KNOW/RESPONSIBILITIES: • Provides physical and emotional care to support the patient and family in periods of crisis through case management with the members of the IDT. • Submits appropriate documentation and paperwork to facilities at the completion of patient visits. • Performs supervisory duties for hospice aides and Licensed Practical/ Vocational nurses and adjusts assignments as needed to promote ideal patient outcomes. • Communicates with IDT, Medical Director and Attending Physician as indicated regarding measures to alleviate symptoms, and monitors response to measures implemented. • Completes routine and emergency assessments on each patient as indicated; may include participation in on-call services to provide holistic team-based approach to hospice services. • Appropriately records observations, interventions and other pertinent information.
