Overview
We are currently seeking a Continuum Care Manager-Inpatient for our Cross Care Continuum Department Required: -Registered Nurse in the State of Illinois required -BSN degree required -BLS/CPR for the Healthcare provider (American Heart Association) or CPR/AED for the Professional Rescuer (American Red Cross) -Minimum 2 years’ current hospital inpatient med/surgical clinical nursing experience -required. -Minimum 2 years’ utilization review, discharge planning, case management or disease management required. What you need to know: --This position requires occasional floating between hospital sites (Evanston, Glenbrook, Skokie, Highland Park). -Facilitate payer authorization process and ensure admission classification is accurate. -Work with physician care team to ensure documentation accurately reflects severity of illness and intensity of service. -Collaborate with physicians, nursing and other members of the multi-disciplinary care team to effect timely, appropriate patient progression. -Address/resolve system problems impeding diagnostic and treatment progress. -Facilitate completion of discharge assessment and plan, modification of plan of care, communication to third party payers and assignment of appropriate level of care. -Facilitate complex care meetings with patient and families to coordinate the plan of care including post- acute care needs.
