Case Manager Supervisor

Overview

Required skills & experience (the 5 “must haves” to be considered) 1. Must have a degree in either: • Social work or a related field. Ideally LSW, MSW, LCSW or LICSW or comparable) • OT or RN with geriatric case management experience 2. At least 4-6 years’ experience working with older adults or adults with a need for some level of ongoing case management, chronic or progressive illnesses and the impact on families/caregivers. 3. Must have knowledge of the elder continuum of care.. 4. Must be comfortable with working within and learning new software and technology tools that support aging in place. 5.
