Overview

Required skills & experience (the 3 “must haves” to be considered): 1. Minimum of 1-year experience in psychiatric health care facility with direct experience in family and group therapies, crisis intervention, and treatment skills; 2. Must have strong clinical assessment skills; Trauma Informed Care, Sexual Reactive, or Substance Abuse Experience. 3. Must be a LCSW, LMSW, LPC, LAC, LMFT, LAMFT 4. Master’s degree in Social Work, Counseling or related mental health field What you need to know: • Provides direct-care services to patients including individual, group and family sessions ensuring programming activity in the milieu. Serves as a primary therapist to assigned patients and their families. • Documenting therapeutic interactions and interventions accordingly in the medical record. Complete and maintain accurate records of services provided and ensure documentation in the patient medical record in a timely manner within 24 hours of delivery of services. • Completes expected documentation within timeframes this includes but not limited to: psychosocial, master treatment plan, group notes, individual notes, all notes. • Completes outcome assessments at admission, during treatment, and at discharge and uses this data to inform the treatment team and submits this data to leadership for cumulative understanding. • Interviews patients and family members to develop a complete psychosocial assessment. Obtains information about patient history, health and level of functioning, and collateral information. • Keeps key stakeholders, with a consent, abreast of treatment progress ensuring the relationships. This can include: families, probation officers, therapist/case worker, school, etc. • Serves as a member of the multidisciplinary team. Participates in the planning, implementation and evaluation of treatment plans and updates. • Works with patients and families to prepare solid aftercare plans, including referrals to community agencies and other professionals as needed. • Ensures program functions within requirements established by the Joint Commission and other state and federal agencies. • Remains knowledgeable of current trends related to behavioral healthcare by participating in professional organizations, reviewing current literature and communicating with representatives of other facilities. • Responsible for creating and supporting a positive, collaborative, professional, team-oriented, harassment-free work environment by understanding and complying with organization policies. • Responsible and support activities related to performance improvement with delivery of care processes and documentation. This includes teaching/training of YCW/MHT/LPN/RN to conduct groups by co facilitating. • Responsible for a collaborative on call schedule and rotation of schedule to meet patient needs seven days a week.
