Oncology Nurse Navigator

Overview

Required: -Minimum 3 years Oncology experience required -BSN required -Nurse Navigator experience preferred -OCN certification preferred -Prior leadership experience preferred -Current professional nursing license in the state of IL required -BLS/CPR Certification required What you need to know: -Must live in close proximity to Evanston and Highland Park (will float to different facilities each week). -Establish initial relationship with potential oncology patient. -Coordinates New Patient Intake. -Coordinate the Tumor Board to help facilitate management of the thoracic oncology population. -Support care standardization for the thoracic oncology population (i.e. standardize genetic Testing/Liquid Biopsies/ process). -Assist patients to streamline their care across the cancer continuum. -Facilitating scheduling of appointments and future testing. -Gather and obtain needed information to prevent delays in confirming diagnosis and access to care. -Ensuring collaboration with multi-disciplinary team to achieve desired outcomes. -Patient and family education.
