Overview

Required Skills and Experience: - You are a Board-Certified (or Board-Eligible within one year of employment) Nurse Practitioner with an unrestricted license to practice in NY - AANC or AANP certification for Nurse Practitioners - Active DEA Controlled Substance Registration or eligible for application - Certified in Basic Life Support for Healthcare Providers - Maintenance of professional credentialing and CME standards - You have experience providing clinical services to individuals with both chronic medical and behavioral health conditions, and have interest in serving complex, vulnerable, and disabled populations What You Need to Know: - Work a full-time 40 hour week, Monday-Friday 9am to 5pm ET with a Saturday half-day rotation and the on-call schedule. - Act as the practice-based provider dedicated to supporting members with walk-ins and urgent or emergent visits in the hub, in order to keep members healthy and in the home - Triage and support members virtually and telephonically based on the member’s preferences and access to technology - Partner with the hub-based clinical and operations teams to create and facilitate an efficient practice that promotes member engagement, trust, and support - Provide comprehensive care management, chronic disease management, preventative care and wellness and liaison with relevant other providers around behavioral health and long term service and support needs, and the provision of palliative care
