Overview
Opportunity for a Home Health RN to work on the beautiful coast of Oregon. Looking for a team player to hit the ground running that has all of the qualifications below and can start working ASAP! MUST have a clear driving record as well as an active driver’s license and current auto insurance! You must provide proof of these 3 items within 48 hours of offer acceptance! Ensures the interdisciplinary care plan is consistent with the patient’s clinical course, continuing care needs and covered services by monitoring diagnostic testing, treatments and procedures, and other aspects of patient care as appropriate for acute care. Discusses with physicians, the appropriateness of resource utilization, consultations, treatment plan, estimated length of stay and discharge plan. Focuses on complex patients, frequent emergency department utilizers, chronic pain and substance abuse patients, and homeless patients. Collaborates with acute care case managers to ensure appropriateness of ongoing care. Coordinates the transfer of patients to tertiary centers, including the transfer of patient information required for continuity of ongoing treatment and services. SUBMISSION REQUIREMENTS 2+ years of Home Health experience – Required OASIS proficient – Required (no exceptions) Able to complete 4-5 visits per day - Required 1 year Case Management experience in Home Health setting – Highly Preferred Recent travel experience – Highly Preferred BSN - Preferred Case Management Certification – Highly Preferred Mileage: .43 centers per mile 🎌AS OF 3/13/20, A COVID-19 ATTESTATION FOR YOUR TRAVELER MUST BE UPLOADED WITH EACH PROFILE FOR THIS NEED.
