Large Catholic Health system is seeking an NP/PA who is ideally looking to join the team in Philadelphia. This position requires a team player who is looking for a permanent position within a model of care that is centered on the well being and health of Seniors. Job Description: Monday - Friday 8:00a - 5:00p Call is evenly distributed among all providers, approximately 5 to 7 days per month as is chosen by the provider. Weekday call 5:00p - 8:00a Weekend Call 8:00a - 8:00a Skills set=general/family practice/adult medicine, independent worker Patient pop = 55+ adults “Adult day care center” with clinic (15 mins – 60 mins appt.) 9:30am – 2pm scheduled, after 2 get through charting, idp meetings in the morning, ~5-8 visits per day. -Evaluation and treatment of acute issues for long term/SNF participants. -Participate in On-call rotation with other PCPs. EMR: TruChart PERM NP Salary Range - $43.05 - $64.57 . REQUIREMENTS: NCCPA or AANP Certified – Required PA License - Required Interest in Perm– Required!!
