Overview

Required Skills and Experience: 1. Active Nurse Practitioner with 5 years' primary care clinical experience caring for patients with complex chronic illness; skill at navigating behavior change in the face of mental illness and social needs 2. A collaborative, empathetic approach to leading care teams that fosters effective and efficient interventions, as well as inspires care team members to achieve ambitious goals and further their own development 3. Strong “builder” mindset, with the ability to continuously assess how our care model can improve as it scales 4. An ability to understand and prioritize patients’ core needs and translate them into well-designed, actionable care plans 5. At this time, employees in this role are required to have completed a first and final dose of a COVID-19 vaccination approved or authorized for emergency use by the FDA, consistent with applicable law. What You Need to Know: 1. This is a full-time position. Seeking folks who reside in any of the following counties: Martin county, Bertie county, Washington County, Edgecombe county 2. Two sides to our model: Digital and Delivery (in home). We provide a more holistic approach to patient care whereas it's not about how many folks you see in a day, but the quality of care that is more important. 3. Provide exceptional, personalized care to cohorts of medically complex, underserved Medicare/Medicaid patients 2. Drive ongoing iteration of our clinical model, with a focus on improving outcomes while decreasing wasteful costs 3. Partner with Galileo’s clinical specialists around conditions that have the greatest impact on our patient population—particularly behavioral health 4. Utilize and inform the development of new digital tools for patient communication, team collaboration, and clinical decision support 5. Provide care in a variety of settings-clinic, mobile airstream clinic, home, video

Qualifications

• Master's degree or higher
• Valid NP in North Carolina (NC)
• 5+ years of experience in ANY of the following:
  • Primary Care
  • geriatrics
