Overview
Required skills & experience: • Unrestricted Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) • 2+ years of clinical experience as a LCSW or LPC • 1+ years management/supervision • Comprehension of the principles and practices of behavioral health treatment, including assessment, diagnosis, treatment planning, cognitive-behavioral and brief intervention counseling strategies and their application to population-based care for preventive and pervasive mental health issues. • Skill and expertise in conducting group and individual psychoeducational classes and therapy strategies that will help strengthen the patient’s overall physical and mental health • A background in maternal/substance use populations • Reliable transportation and valid driver’s license • Advanced computer proficiency; proficiency with using Cloud-based systems, EMRs, and teleconferencing applications • Strong understanding/interest in public health and population health • Familiarity with Medicaid/Medicare/Commercial Insurance practices • Previous experience working from home and organizing daily and weekly tasks What you need to know • Deploy standardized clinical model to achieve clinical outcomes that align with financial outcomes • Lead and manage a team of Partner Advocates, Peer Advocates, and Nurses with the responsibility of overall team success • Aid in the construction of clinical pathways, group therapy, classes and behavior-focused practice protocols • Conduct biopsychosocial assessments to determine behavioral healthcare treatment plans for patients • Lead interdisciplinary team meetings providing clinical assessments and measures to coordinate treatment considerations • Lead case conferences, supervise and help manage complex cases that require a holistic clinical management approach • Implement psychoeducation for patients during individual or group visits • Provide same-day feedback to primary care provider and or care team • Responsible for achieving goals/Key Performance Indicators (KPI's) for region • Expedite and supervise referrals to specialty behavioral health services, and when appropriate supporting a smooth transition from special behavioral health services to primary care • Accountable to understanding team roles and how each affect utilization management benchmarks and quality outcomes • Engage in on-call crisis line management
