Required skills and experience: 1. Experience in a service environment. 2. Compassionate, kind, and open-minded. 3. Teamwork experience. 4. Written communication skills. 5. Some clinical experience is a plus, such as EMT, MA, CNA, or Phlebotomy certification. What you need to know: 1. This is a full-time opportunity with a young organization with competitive benefits package! 2. There's an operations aspect to the job when you assist with the coordination of care across the continuum, such as: scheduling appointments with specialists, coordinating referrals, and sharing or transferring information with the patient’s internal and external care team. 3. There is a clinical aspect to the job where you will greet patients and families in reception, bring them back to rooms, establish an agenda, and work with the patient to ensure a pragmatic care plan is established. 4. There is a coaching aspect to the job where you will communicate with patients by phone and email and invite in the physician when necessary and help patients understand their chronic conditions, why they matter, and how they can manage them.
