Overview

Required skills and experience: 1. MS degree in social work from an accredited school of social work approved by the Council of Social Work Education. 2. Current State License as a Medical Social Worker, or LIMSW. 3. A demonstrated ability in casework, counseling and group work, including 2 (two) years of clinical work experience, preferably in either a healthcare setting or hospice. 4. Social work skills as defined as generally accepted social work standards. Requires excellent verbal and written communication skills with strong interpersonal skills. Knowledge of community and community resources. Knowledge and understanding of hospice philosophy and regulations. Ability to work independently. 5. Reliable transportation. 6. Meets all applicable agency policies and procedures related to health screening and required testing. What you need to know: 1. This is a full-time opportunity, Monday - Friday 8a-5pm. 2. Assesses patients to identify the psychosocial, financial and environmental needs of patients as evidenced by documentation, clinical records, case conferences, team report, call-in logs, and on-site evaluations. 3. Counsels, instructs and includes the patient, caregiver and family in following the Plan of Care which may include signs & symptoms of decline in status and final stages of impending death. 4.
