Overview
Required skills & experience: • Graduation from high school or equivalent required • Certified Nursing Assistant (CNA) certification required, Home Health Aide (HHA) certification • Current Basic Life Support (BLS) / First Aid Certification required • Bilingual preferred (Spanish) • At least 1 year of recent experience providing personal care to a frail or elderly population as a CNA or HHA • Ability to process accurate information in a timely manner • Excellent organizational and communication skills • Ability to follow directions from supervisor and communicate needs effectively • Experience and competency working with people from diverse backgrounds • Deep commitment to helping unlock the potential of our most vulnerable seniors What you need to know: •Provide personal care and assistance with ADLs (activities of daily living) to the PACE participants, including but not limited to escorting, toileting, bathing, dressing, grooming, assisting with meals, laundry, etc. •Measure and document participants’ vital signs as instructed. •Assist participants with boarding and exiting from the client's PACE transportation vehicles and acting as medical escort when instructed. •Observe activity on the day center floor to assure participant safety. •Assist with meal service, ensuring participants receive the correct meals and providing support with feeding when indicated. •Maintain personal care supplies such as incontinence supplies and personal grooming items. •Provide translation services as needed, if qualified. •Assist Engagement team and Home Care team as needed. •Clean participant areas in the day center between meals and activities. Schedule: 5 days a week. M-F 8-4:30 (Could vary if participants have early or later appointments – earliest start time would be 7am and latest being 9am)
