Original Content: DESCRIPTION: We are actively recruiting a PRN Pharmacist to join our Population Health Team. This position will be involved in the clinical management of CKD & ESKD patients. The candidate will be a critical member of the Population Healthcare team consisting of nurses, dietitians, social workers, community health workers, nurse practitioners and physicians. You will conduct medication reviews to identify and address medication discrepancies and improve care coordination for our complex renal patients with multiple chronic conditions and polypharmacy.  This role will be expected to provide patients with medication education and tackle any financial concerns to medication costs. Your primary focus will be to improve patient outcomes through ensuring patients understand/adhere to their medication regimen, and have access to affordable medication RESPONSIBILITIES: •• Transitional care medication reviews. • Kidney care medication reviews. • Answer medication-related questions. • Outreach to nephrologists, primary care providers, and dialysis center staff for medication coordination. • Work collaboratively with care team members to address patient's social and medical needs holistically. • Articulate various issues related to the drug process. • Ability to advise patient care team to evolve from a disease-oriented approach to a patient-centered approach with focus on prevention. • Knowledge of federal and state laws. • Familiarity of Insurance company guidelines. • Ability to counsel patient and reduce health disparities through cultural competence and promoting diversity. MEASURES OF SUCCESS • Provider engagement and collaboration • Number of medication reviews conducted • Performance scorecard QUALIFICATIONS: • Analytical Skills • Medication dispensing experience • Good judgment • Written & verbal communication • Pharmacology • Managing processes • Legal compliance • Productivity • Quality focus • Attention to detail Required/Preferred • 1-2 years in clinical practice. • Doctorate Degree in Pharmacy (PharmD). • Care Management Experience. • Demonstrates empathy, enthusiasm, and a strong work ethic. • Proactively acts as a patient advocate and responds with resolve. • Knowledge and experience to empower patients in self-management and shared decision making. • Experience working with a vulnerable patient population (ESRD, geriatrics, minorities, low income, uninsured, etc.). • Enjoys working collaboratively with team members. • Familiar with electronic medical records. EDUCATION, EXPERIENCE AND LICENSING REQUIREMENTS: • Pharmacy degree from an accredited pharmacy school recognized by the American Council of Pharmaceutical Education (ACPE). • Valid state pharmacist license (or must meet education requirement(s) for state licensure). • Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification upon hire.
