RN Care Coordinator: Home Recovery Care

Overview

Required skills & experience: 1. ASN required, BSN preferred. 2. Currently licensed as a RN in WI. 3. 3-5 years of inpatient experience (preferably in med/surg) as a registered nurse is required and within the last 5 years required. 4. Experience providing nursing care in the home, community, or clinic is highly desirable. 5. Prefer candidate to have Certification: Care Coordination and Transition Management (C.C.C.T.M) What you need to know • As a Recovery Care Coordinator (RCC), you will be at the forefront of Contessa’s new and highly innovative care model that allows patients who are clinically the option to receive acute level care in the comfort of their own home in lieu of an inpatient setting. • Schedules: 30 hours a week with rotating on call • Mileage reimbursement: IRS standard - looks like it will be approx .56 p/mile • Facilitates communication and coordination between all members of the care team. • Assists all patients throughout the care model by acting as a patient advocate and navigator. • Provides patient education of program including telehealth education and disease specific patient education to patients.
