Overview
Required skills and experience: •You have a current, unrestricted Massachusetts RN license •You have 3+ years of experience providing clinical services to Adult and Geriatric individuals with co-occurring chronic medical and behavioral health conditions. Ideally 4 years. •Experience as a Home Health RN is very desirable. Must be comfortable with home visits, community visits, virtual visits and eventually clinic visits when we open a clinic in the Massachusetts market •Experience completing MDS assessments •Working on interdisciplinary care teams (Team also has social worker/case manager profile, Provider (NP/PA), BHS (LICSW) •Unrestricted Massachusetts Driver’s License and Car What you need to know: • You will go above and beyond to connect with members including, but not limited to Lowell, Boston, and surrounding communities In Middlesex and Suffolk counties, Massachusetts community in a non-judgmental, respectful and empathic manner, to meet their needs, and to provide feedback to the system as a whole as we strive to do better every day. • Assess Adult and Geriatric members to provide accurate clinical and holistic depiction of the member at their Initial and Annual assessments, as well as at any point of care in which the member may have had a change. Two Locations in territory: •Middlesex County - concentration of members in Lowell, MA but some members in neighboring towns/communities •Suffolk County - most of these members are in the city of Boston/Boston proper with some members in the neighboring communities in the Greater Boston area ***Personal car is required for work travel and Driver’s License. Schedule: M-F, 9am-5pm Saturday Rotation: 1x every 5-7 weeks you will work a Saturday morning shift (9am-1pm AKA a half day). When you work the half day, you get back half a day for a weekday
