Overview

Required skills and experience: 1. Active RN license in CO. 2. Critical thinking and judgment in the provision of clinical care. 3. 3+ years of nursing experience in hospital or ambulatory setting. 4. Flexibility and comfort with a young organization that is transitioning from its early start-up phase to one with more structured processes. 5. Spanish Speaking preferred. 6. Phone triage experience required. What you need to know: 1. This is a full-time opportunity to work with a start-up transforming health care, starting with primary care. 2. Need to have comfort with MacBooks, Google software, and modern communication technologies. 3. Schedule: Monday - Friday 8a-5pm. 4. Our care model changes everything - the team, outcome-focused payment, customer service, and the technology that supports our care. Additional Job Duties: 1. Provide direct patient assessment and care for adults 65 and older. 2. Phone and walk-in triage for patients. 3. Perform phlebotomy, IV initiation, and drug therapy management. 4. Administer IM/SQ/and PO medications and vaccines. 5. Provide treatments and assist providers with procedures and minor surgeries. 6. Perform diagnostic tests (including but not limited to: EKG, ABI, Spirometry). 7. Oversight of in-house laboratory processing. 8. Perform point of care testing. 9. Provide patient teaching, education and support. 10. Participate as an active care team member across multiple disciplines. 11. Participate in the clinical development of Health Coaches. 12. Participate in process and quality improvement initiatives. 13. Visits to patients in hospital, skilled nursing facility or at home. 14. Oversight of anti-coagulation clinic activities and patient monitoring. 15. Wound care. 16. Transitions of care coordinator (post-discharge visits, discharge calls…). 17. Complex care coordinator (multimorbid or high-risk patients).
