Required skills and experience: ●At least 2 yrs of experience leading a team as a Program manager ●Master’s Degree, preferably in a healthcare-related field ●Ability to travel within assigned region to meet with team and members as needed ●A Qualified Mental Health Professional (QMHP) holding one of the following licenses: Licensed Clinical Social Worker (LCSW), Licensed Clinical Professional Counselor (LCPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Psychologist (PhD or PsyD). Additional job details: ●Seeking a Program Manager in our Population Health Program to lead a team of Care Extenders and Case Managers in Lexington, Kentucky. ●An innovative Program Manager seeks creative solutions to support Care Extenders and Population Health initiatives, aligning with program goals and driving toward the desired outcomes. ●As the leader of a community-based team, they model a hands-on approach, empowering the team to meet members where they are, and tapping into the wide range of local resources to deliver whole-person care. ● M-F schedule & 50% remote 50% in field for travel
