Overview
As an Intake Admission Specialist, the work you do every day makes a difference in the lives of our patients by communicating with referral sources, staff, and community resources to transition patients to home care services. HOW YOU'LL WORK: You’ll be responsible for inputting all patient information into our system accurately. You will also build relationships with community physicians and other referral sources. MAJOR AREAS OF RESPONSIBILITY: • Operations: Process all daily patient referral and intake operations. • Communication: Work with multiple service providers to coordinate care and call physicians to obtain orders when needed. • Customer Service: Follow up with patients in regard to Start of Care (SOC) & services being provided to measure satisfaction. • Troubleshooting: Follow up and report delay stages, non-admits, and any other SOC issues. • Policies: Understand and practice agency policies and procedures • Collaboration: Establish and maintain positive working relationships with current and potential referral sources. Provide support to Community Educators and or Community Liaisons referral sources and clients. • Compliance: Perform workflow and data reporting in accordance with CMS guidelines and ensure compliance with all state and federal referral/intake regulatory requirements. REQUIREMENTS • Associates degree or equivalent healthcare/home care experience required • Two years of healthcare experience with one year in home care is preferred • Experience in Medical Terminology is preferred • Knowledge of government regulations and private payer practices • Capable of all physical demands.
