RN Care Manager

Overview

Required skills and experience: •Active, unrestricted VA or DC Registered Nurse license •Minimum 3 years of experience as an RN (**Ideally 4 years) providing clinical services to Adult and Geriatric individuals with co-occurring chronic medical and behavioral health conditions. •Experience as a Home Health RN is very desirable. •Must be comfortable with home visits, community visits, virtual visits and eventually clinic visits when we open a clinic in the Massachusetts market •Working on interdisciplinary care teams (CHP - social worker/case manager profile, Provider (NP/PA), BHS (LICSW) •Hands on clinical care - we can do a review/training on some things listed above, but not all. What you need to know: •We employ a field-based, home-based care model and are committed to meeting members where they are--in their homes, in their community, and in our Hubs. You will go above and beyond to connect with members including, but not limited to the Washington, DC area. ***Personal car is required for work travel and Driver’s License. Schedule: •Hours: M-F, 9am-5pm •Saturday Rotation: 1x every 5-7 weeks
