Overview

Required Skills and Experience: 1. Valid New York State driver’s license. 2. Licensed Clinical Social Worker degree with active license in the state of New York; Master’s degree in health care or social work preferred. 3. Working knowledge of Medicare and Medicaid programs and experience with regulatory requirements and reporting. 4. Knowledge of managed care and / or member services. 5. Knowledge of community services and resources for elders and disabled. 6. Familiarity with coordinated care models is an advantage. 7. 2+ years’ experience in a member services department, health care organization, Community Based Organization, or social services environment. 8. Demonstrated experience in a managed care or HMO setting is strongly preferred. 9. Three (3) to Five (5) years of progressively responsible in a high-volume call health care or social service related. What You Need to Know: 1. This is a full-time opportunity with competitive compensation package including a 10% annual performance-based bonus incentive. 2. Work will be 40 hours per week with a mixture of shifts between 8AM-7PM with some weekend coverage as well. It’s scheduled shifts though so most hours will be during business hours. 3. Mileage will be reimbursed as this is a field position and candidates will meet in patients’ homes and in the community. 4. The position will be heavier on the social work and lighter on mental health. Skillset in a Qualified Candidate: 1. Understanding of the needs and dynamics of elder care services, disadvantaged, disengaged populations. 2. Experience in participating in cross departmental projects and policy and procedure changes, including coordination of activities and initiatives across departments. 3. Demonstrated ability to work in a fast-paced, high volume call-center environment and manage staff to both meet and exceed goals and key performance measures. 4. Passionate about customer service with a continuous improvement mindset. 5. Excellent problem solving, trend identification, and analytical skills, with a demonstrated ability to problem solve effectively and efficiently. 6. Commitment to high ethical standards in all work; protects the privacy of member and Belong Health data and exercises discretion in handling confidential member information. 7. Excellent oral and written communication skills, including presentation skills. 8. Demonstrated PC skills using Microsoft applications. 9. Must be able to travel to other office locations, as necessary. 10. Must be able to work flexible schedule. 11. In depth knowledge of managed care product portfolio, benefits, policies and procedures. 12. Strong active listening, issue assessment and problem-solving skills. 13. Strong organizational and follow through skills. 14. Ability to exercise discretion in handling confidential member information. 15. Passion for customer service and process improvement. 16. Qualifications

Required
  • Master's degree or higher
  • ANY of the following valid licenses/certifications:
    • Licensed Master Social Worker (LMSW) in New York (NY)
    • Licensed Clinical Social Worker (LCSW) in New York (NY)
  • 2+ years of experience in ANY of the following:
    • Community Based Care
    • Medicare/Medicaid
    • Home Health Care
    • high-volume call health care or social service
