Overview

Required skills & experience (the 5 “must haves” to be considered): • Master’s degree with Nurse Practitioner certification in relevant area; Geriatrics, Adult or Family Nurse Practitioner • Current unrestricted Nurse Practitioner Licensure in state of employment • Experience in Geriatrics, Long Term Care, Hospice, or Palliative Medicine required (2-5 years preferred) • Ability to work in a collaborative atmosphere, focus on teamwork, flexibility to change and adapt to a rapidly changing and evolving program. • Intense intellectual curiosity and an ability to view old problems with a fresh perspective • 2-5 years of experience in above relevant areas • Experience and comfort with electronic medical record documentation, basic computer skills, email, conferencing What you need to know: • Searching for Nurse Practitioner Care Managers to join our new Institutional Special Needs Plan team (I-SNP). In this position, you will assist with the development and execution of the clinical model for the I-SNP members in collaboration with the Primary Care Provider. You will provide care management and medical care to elderly individuals who live in nursing homes. The NP Care Manager will partner with the Primary Care Provider to provide the highest quality of care to I-SNP members. • Location: Greenville, SC • Start Date: As soon as credentialing, drug and background clear • Duration: 12 Months (January 2022-December 2022) --Will train and slowly pick up patients until full caseload 2 months later. --Need time to get acclimated into nursing homes.
