Our client is a Hospital and Health Care company that provides virtual behavioral health, and on-site pain management, physiatry, and other specialty care for adults living in skilled nursing facilities and other congregate living communities. Their Virtual Behavioral Health Clinic, supporting all adults and caregivers, is now open to anyone 18 and over.

Required
  • Master's degree or higher
  • Valid Psychiatric-Mental Health Nurse Practitioner (PMHNP) in Texas (TX)
  • Geriatric Care (2+ years)
  • Skilled Nursing Facility Care (2+ years)

Overview

Required skills and experience: 1. Must be fully vaccinated. 2. Current, unrestricted TX PMHNP license. 3. PMHNP certification from an accredited program. 4. Minimum of 1 year clinical experience as a PMHNP, with home-based care desired. 5. Knowledge of CPT, ICD-10, and HCPC codes. 6. Bonus points: Awareness about UM standards, NCQA requirements, CMS guidelines, Milliman guidelines, and Medicaid/Medicare contracts and benefit systems. What you need to know: 1. This is a full-time Telehealth remote opportunity, 40 hours a week 2. Up to 10 telehealth patient visits per day. 3. Can be located anywhere within the state of Texas! 4. No on call or weekends! Additional job details: 1. Treat patients via telemedicine in Assisted Living Facilities, Skilled Nursing Facilities, and Independent Living Facilities and periodically be available for in-person visits. 2. Actively participate with interdisciplinary team meetings to assure behavioral health issues are considered and addressed for all patients. 3. Establish and maintain effective therapeutic relationships with members and their legal representatives, facility staff, PCPs, and specialists to maximize continuity and compliance with individualized member care plans. 4. Perform an initial comprehensive assessment on all newly enrolled members and at least annually thereafter. 5. Develops a member management care plan upon enrollment and updates it as needed when changes in condition warrant or following hospitalization.
