Overview

Required skills and experience: 1. Active and unrestricted Registered Nurse and Nurse Practitioner license for the state of MS. Board certified by ANCC or AANP. 2. Current and unrestricted DEA certificate. 3. Ability to work without direct in-person supervision and practice autonomously. 4. Willingness to travel daily; access to transportation, a valid driver's license, and car insurance. 5. Must be proficient with medical instruments and equipment required by the work. 6. Knowledge of computer-based data management programs and information systems, as well as medical records and point-of-interview technology. 7. Ability to communicate effectively, in verbal and written form, with retail and medical partners at various levels, patients, family members, physicians and representatives of the community. 8. Sound understanding of all federal and state regulations including HIPAA and OSHA. 9. 2 or more years of NP experience preferred. 10. Bilingual in Spanish is helpful. 11. Experience in kidney disease and control of related co-morbidities (HTN, Diabetes, CHF) preferred. 12. Experience in end-of-life discussions, palliative care and hospice preferred. 13. Experience in practicing in managed care, value-based context is necessary. 14. Experience conducting annual wellness visits or similar comprehensive visits and appropriately charting HCCs; general knowledge of risk adjustment. Benefits: 1. Deliver evidence-based, timely care in a manner that reduces avoidable hospitalizations, maximizes quality of life, and puts patient health and satisfaction first Conducts assessments, which includes comprehensive annual wellness exams on patients. (Assessments will occur both through in-home patient visits and through telemedicine.) 2. Prescribe medications, order tests, and collaborate with patient’s Monogram physician. 3. Documents items such as: chief complaint, all applicable diagnosis, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan. 4. Collaborate with care management RNs and SWs to ensure consistent, highly satisfying patient experience, accurate documentation, and effective care planning. 5. Complete all documentation and paperwork in a timely manner. 6. Maintains quality of care and productivity standards as defined by the practice. 7. Perform effectively, as reflected by improved patient quality outcomes, which will be measured and reported daily. 8. Telehealth approach to visiting patients will be part of this job.

Qualifications

Required
  • Valid NP in Mississippi (MS)
  • NP (2+ years)
  • 2+ years of experience in ANY of the following:
    • Home Health Care
    • Hospice
  • Managed Care (2+ years)
  • palliative care (2+ years)
  • Dialysis Care (2+ years)
  • Do you have experience with computer-based data management programs?
