Qualifications

Required
  • Master's degree or higher
  • Valid FNP or AGNP in Louisiana (LA)
  • Valid state Driver's
  • 1+ years of experience in ANY of the following:
    • FNP or AGNP
    • senior care
  • Are you a Board Certified Nurse Practitioner?
Preferred
  • Do you have a reliable form of transportation?

Overview

Start date: January 4th, 2022. Contract to start at 6 months and can be extended from there. Required skills and experience: •Board Certified (FNP or AGNP) •Active State License. •Minimum of 1-year experience as an NP. •Must be tech-savvy and be able to easily utilize an array of applications, a smartphone, tablet, and EMR. •Background Check/Active medical license with a check is required. •Full COVID-19 vaccination is an essential requirement of this role. Preferred Qualifications: •Active and unrestricted DEA license. •Medicare Provider Number/ Medicaid Provider Number. •Knowledge of HEDIS measures and the ability to close gaps in medical care. •Strong work ethic with effective communication skills and a collaborative spirit. •Attention to detail, especially in medical documentation. What you need to know: •We are hiring full-time and part-time nurse practitioners to perform in-home health risk assessments for seniors. •This is an autonomous field based, prevention/education focused opportunity that takes you out of the mundane office setting. •Our healthcare providers conduct comprehensive, in-home health risk assessments with patients as part of their health insurance benefits. •The evaluations are conducted in a face-to-face manner where the member resides. •Within the visit, the clinician reinforces the importance of staying current on covered preventive services while identifying potential gaps in care, helping to prevent unnecessary hospitalizations and ER visits. Schedule: 4 days a week, 10 hr shifts. No weekends required, but NP can work on weekends if they prefer. Start date: January 4th, 2022
