Overview
Required skills and experience: · At least 2 years of outpatient experience in full-spectrum family practice · Graduate of an accredited program for Nurse Practitioners (specifically FNP) · Advanced education and training in the diagnosis and management of common medical conditions and chronic illnesses · Current licensure as a Nurse Practitioner in state of Pennsylvania and/or New Jersey · Current Certification by the American Academy of Nurse Practitioners · Meets all requirements in order to obtain associate medical staff membership and required privileges of service · Current DEA and NPI # · Experience in an urgent care environment a plus · Experience in a start-up environment a plus · Bilingual (English and Spanish) a plus · Background in public health a plus What you need to know: Position Description: Lead clinical operations and supervise other clinicians providing care in a unique, proactive care model that resembles a hybrid of primary care and urgent care for low-income communities. The ideal candidate will be focused on the delivery of quality care in accordance with the company’s culture of outreach and support. This candidate will be responsible for conducting patient visits related to acute, chronic care and preventive care management. They will assist with the development, implementation, management, evaluation and achievement of specific programs, site objectives and initiatives, including coding for quality measures. Of utmost importance, this role is for an individual comfortable in a startup environment where an “all hands-on deck” and “operating in the gray” mentality is key in fostering a positive team feel in a time of great growth. Some Essential Job Functions Include: · Supervision and support of all clinicians providing care in the clinics. · Development of protocols for initial evaluation, history, physical examination and treatment of patients whether they are primary care, urgent care or complex care patients · Work closely with Data Team to improve processes regarding collection and use of appropriate data to assist in the proactive treatment and management of patients · Enhance procedures and programs around the appropriate management and treatment for the initial evaluation of a problem or the follow-up of a previously diagnosed condition · Develop protocols and processes for patient care management within advance practice parameters; manage acute care and chronic care activities · Collaborate with Director of Operations to develop processes for inbound clinical triage and follow-up · Create programs to assist with patient education activities · Conduct presentations and community sessions pertaining to disease specific process and wellness · Monitor the quality of care delivered to our members · Document patient contacts appropriately; complete chart reviews on a regular basis and in a timely manner · Provide and receive feedback on quality of care being delivered to ensure JCAHO and other regulatory standards are met
Qualifications
- Master's degree or higher
- Valid Nurse Practitioner (NP) in Pennsylvania (PA)
