Overview
Required skills & experience: 1. ASN required, BSN preferred. 2. Currently licensed to practice in AZ. 3. 3-5 years of inpatient experience (preferably in med/surg, ED or ICU) as a registered nurse is required and within the last 5 years required. 4. Experience providing nursing care in the home, community, or clinic is highly desirable. 5. Prefer candidate to have Certification: Care Coordination and Transition Management (C.C.C.T.M), but not required. 6. Covid-19 vaccination required. What you need to know 1. As a Recovery Care Coordinator (RCC), you will be at the forefront of our new and highly innovative care model that allows patients who are clinically the option to receive acute level care in the comfort of their own home in lieu of an inpatient setting. 2. Schedule available for St. Joe's Central Valley: 11-7p w/ Rotating holidays and on call. 3. Mileage reimbursement: IRS standard - looks like it will be approx .56 p/mile. Additional Job Details: 1. Identifies potential patients for program inclusion through rapid recognition of clinical determinants that indicate patient eligibility. 2. Possesses clinical knowledge, experience, and acumen to identify, present, and discuss potential HRC patients to providers for program inclusion. These direct discussions usually include ED providers and hospitalists but may span across multiple subspecialties. 3. Facilitates communication and coordination between all members of the care team to coordinate admissions. 4. Coordinates referrals and appropriate resources to assist patient and/or caregiver in continuation of care in the outpatient setting. 5. Generates operational quality reports and presents updates on to our partners' physicians and nurses. 6. Maintains all required documentation in all interactions with our health systems' patients and the care team. 7. Follows clinical and operational workflows. 8. Provides prompt, courteous, and excellent service to internal and external customers. 9. Interacts with the patient and the multidisciplinary team to coordinate the services ordered by our partners' physicians. Communicates to the appropriate providers about any barriers to the patients' fulfillment of our partner provider's Care Plan. 10. Communicates discharge information to other clinical departments or members of the Care Team. 11. Watches for trends and hurdles involved in health care system and incorporates operational solutions for system challenges, including patient, family and physician responses into an evolving process and model that increases quality and satisfaction- patient or physician. 12. Builds and maintains collaborative professional working relationships with physicians, Medical Directors, clinicians, and community at large to develop and implement a successful cross-continuum care management process 13. Works with partner physicians to educate patients on the Home Recovery Care model; provides education regarding use of the telehealth system and processes.
Qualifications
- Valid Registered Nurse (RN) in Arizona (AZ)
- Are you fully vaccinated?

- Medical/surgical
- Emergency department
- Acute care hospital
- ICU
