Responsibilities As a Patient Advocate work directly with patients to navigate their individual care plans, and identify and reduce any barriers that they may face in receiving quality and timely care. Patient Advocate work directly with medical providers and other multidisciplinary team members to directly coordinate patient care. The Patient Advocate provides outstanding customer service and maintains a compassionate and welcoming atmosphere. The Patient Advocate provides effective customer service by exploring the needs and priorities of patients and office offerings. The Patient Advocate will provide effective customer service for all internal and external customers by using, excellent, in-depth knowledge as well as communicating effectively with team members. The Patient Advocate will develop trusting relationships with the patients and be the primary contact for your assigned patients. Identifies patient care requirements and goals by establishing personal rapport with patients and other persons in a position to understand care requirements. Establishes a compassionate environment through interactions with patients and caregivers. Documents patient interactions in healthcare systems. Monitors clinical status of patients (e.g., medication adherence and referral follow-ups) through dedicated EMR and software. Maintains continuity among staff by documenting and communicating actions, irregularities, and continuing needs, and escalates necessary information to medical providers. Maintains patient information confidentiality and privacy. Maintains a cooperative relationship among health care teams by communicating information, responding to requests, building rapport, and participating in team problem-solving methods. This is a strong multi-disciplinary team environment. Assists physicians and clinicians in orienting patients to their care plans and the network. Problem solves questions from patients or staff (e.g. problems with pharmacy, insurance, prior authorizations). Assists with specialty projects pertaining to the overall improvement and quality of care. Identifies and socializes opportunities to improve care journeys for patients Physically works from one assigned center when provider is present to provide real-time assistance to patients for identified care needs. Maintains safe and clean working environment by complying with procedures, rules, and regulations; calling for assistance from health care support personnel. Assists patients in selecting products. Suggests improvements & recommends solutions. Assist in offering product solutions. Demonstrates superior product knowledge; accurately describes the features & benefits of all products using the appropriate tools. Strives to achieve exceptional results on goals & competencies. Serve as primary point of contact for members as they engage the service - this includes inbound and outbound. Educate member on services within the membership as well as benefits members are entitled to through their health plans. Help members with scheduling and communicating with their care team - including primary care, specialty care, and urgent care providers. Help members with medication adherence via reminders and best practices Connect patients with community resources as needed. Maintain a record of patient interactions in electronic health record. Assist members with accessing their care team via technology, including via our app and telemedicine. Consults with XXX as needed. Responsible for coordinating delivery date and time, set expectations for pick up, seeks feedback about experience and asks for referrals. Develops and maintains a professional business partnership with the XXX.
