Required Skills and Experience: -2 + years as a NP or PA working in primary care - Active Nurse Practitioner License - FNP or AGNP or PA license - Technically savvy and comfortable using tools such as iPads for charting and HIPAA secure messaging apps for care coordination - Must have active NY state license and DEA - Senior Patient Care Experience Required What you need to know: - Typically see one to two patients an hour, with goals set to drive interpersonal relationships with patients and families - A dedicated driver provides transportation while you maintain a medical supply kit - Use of iPad for EMR charting - Client operations support for prescription refills, lab follow up, and coordinated care management

- Master's degree or higher
- Valid Nurse Practitioner (NP) in New York (NY)
- FNP (3+ years)
- Senior Care (3+ years)
- Valid FNP or AGNP in New York (NY)
