Overview

Qualifications for this role: • 2+ years of experience providing peer support to patients with Complex and Multiple Chronic Conditions and challenging Social and Mental Health Conditions (e.g. Community Health Worker, Patient Navigator, In-Home Support Specialist, etc.) • Customer Service or Sales experience with a passion to close • High School Diploma or equivalent (required); some college (preferred) • Computer proficiency and software skills (required); CRM experience (preferred) • Valid Driver’s License • Reliable mode of transportation with proof of auto insurance • Covid Vaccination (required) •.Bilingual in English/Spanish or Korean/French Selling Points: • Full Benefits • Bonus Opportunities • 401K • PTO, Sick Days • Reimbursed for milage • Provided all equipment for daily meetings with patients and providers (iPad, Smart Phone, etc.) • Ability to join a growing company *Schedule: Fulltime | Monday - Friday, Core Business Hours • And much more!

Qualifications

Required
  • High School/GED
  • Valid Driver's in New Jersey (NJ)
  • Do you have a reliable mode of transportation with auto insurance?
  • Are you fully vaccinated for COVID-19?
  • Are you willing and able to provide proof of full vaccination to HR at least 1 week prior to your start date?
  • Are you willing and able to meet the local travel requirements to patient homes?
Preferred
  • 2+ years of experience in ANY of the following:
    • Providing peer support to patients with Complex and Multiple Chronic Conditions and challenging Soci
    • Experience in a Sales related role
    • Experience in a Customer Service related role
