The MA’s can be from anywhere – just have to be willing to work PST time zones – definitely until 5 PST (8 EST), but likely that some of the member availability would be 5-8 PM PST – so potentially until 11 EST and 10, 9, for Central and Mountain time zones. CareScout & Clever Care need Medical Assistants to complete Plans of Cares for members that are part of the Medicare Advantage Program, these members recently had a Health Risk Assessment completed and will need Mas to aide Nurses with putting together a POC by reviewing the HRA (Health Risk Assessment), and speaking with the member for their buy-in. Below are some details/requirements. Their will be a health risk assessments already completed for the members, and from that, you will create the POC (plan of care). You will be speaking with the member to get any additional information to create the Plan of Care. They are said to take about 20 minutes each. POC's will be completed in Clever Care’s system - Problem/Goal, interventions based on the (HRA) Health Risk Assessment - MAs will create/discuss with member to get more details to create the POC - MAs to complete 2-3 per hour **POC tool used is predominately “dropdown selection” versus typing The two disease focuses are: - Diabetes - Cardiovascular disease Additional context: - All the members (patients) are Southern California (LA and San Diego) - MAs can live anywhere - MA can earn $15/per care plan (minimum standard 2 per hour, ideally they get to 3 to 4 as they get good at them).
