Job Title: Registered Nurse Case Manager The Highlights: 1. The opportunity to provide hospice care in home 2. Patient focused approach (22-25 patient visits/week) Necessary Qualifications: - Registered Nursing License in state of employment. - Must have a valid driver's license and auto liability insurance. - Ability to flex schedule to respond to emergency and/or temporary staffing needs for on call needs of the patients. - Must be able to travel frequently Schedule: Full-time Job Description: The Registered Nurse Case Manager is responsible for modeling the 3 Company values of Compassion, Integrity, and Excellence, and for promoting the Hospice philosophy, using the 6 Pillars of success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the Program, between the Program and other Company departments, and between the Program and all external stakeholders.
