Overview
We are seeking an experienced Nurse Care Manager to join our team. In this role you’ll be providing oversight, guidance, and support for care plans, as well as using your nursing assessment and evaluation skills to help guide treatment and care decisions. You’ll also identify services and vendors and navigate patient benefit plans. Our ideal candidate is someone who is able to work collaboratively with our staff, and with patients and their families. He or she will also ensure that medical services are managed in the most effective and appropriate way, and will be responsible for developing and implementing a plan or care. Other duties include completing initial assessments for new members, and monitoring them throughout the care process. This position does require traveling to various residences to check on patients. In this position, you will be joining an organization that is committed to transforming the home health industry. Current RN License, BSN preferred 2. 3-4 years of clinical practice in a hospital, clinic, home care, or nursing home setting 3. 1-2 years of utilization management experience a must 4. Case management experience desired 5. Disease management experience useful 6. Physician office experience helpful
